Registration for the SHOFAR KRAKOW Mission occurs in three parts. Please read the directions below carefully.

STEP 1: Fill out the mission application.

• Download the printable application

STEP 2: Sign release form and submit photocopy of passport.

Download the printable release form. A scanned copy or photocopy of your passport should accompany this release form.

STEP 3: Pay for your mission registration.

Note: There are no refunds on the deposit or any of the payments.
