Online Stock/Mutual Fund Gift Intention Form

This form serves as notification of your intention to make a gift of stock to JCF. It is important that we are notified prior to every stock transfer, in order for us to accurately identify, allocate, and acknowledge your gift.

Note that this form cannot be used for donating restricted stock or stock traded on non-U.S. markets; as well as creating life income gifts or transfer of physical stock certificates.
